Parental Affirmation of Commitment
I, M, and I, L, in the presence of the undersigned Bet Din, hereby affirm that the conversion of our child David to Judaism has taken place in accordance with our express wish and at our specific request.
We jointly pledge to rear our child according to the teachings of the Torah and the practices of Judaism and in no other faith. We undertake to provide our child with a formal Jewish education, supported through our affiliation with a Jewish community, attendance at religious services, and through the creation of an atmosphere of Jewish living and religious observance in our home.
He has already undergone the halakhic requirement of hatafat dam brit for the sake of conversion and thus entered into the brit of our ancestor, Abraham. We accept the above obligations as our child, David, is converted to Judaism this day with the ritual of tevilah, immersion in a mikveh.
We understand that at the age of religious majority, our child has the opportunity to reaffirm the decisions we have made for him. May God help us to raise our child to appreciate the beauty of Judaism and to lead him to a life of Torah, huppah, and ma’asim tovim, good deeds.
